Congratulations!  We welcome you and your family to St. Thomas Aquinas for the celebration of the Sacrament of Baptism. 

BAPTISMAL REGISTRATION
Please complete this registration form prior to the next steps:

DOCUMENTS REQUIRED FOR BAPTISM

1. St. Thomas Aquinas Baptismal Registration Form (link above)


2. Copy of the child’s birth certificate


3. If parents are not registered members of St. Thomas Aquinas, a letter of permission for baptism from the home parish


4. Copy of one of the parent’s baptisms certificate


5. Baptism prep class certificate of attendance for both parents and godparents


6. Godparent Covenant form for both godparents

Please contact the ministries office to confirm the date for the child's Baptism. All Baptism documents must be received in the office before a Baptism date can be set.

Godparents:
Godparents must be practicing Catholics living according to the norms of the Church, if married, must be in a valid marriage. Married couples serving as godparents must be married by the Catholic Church. Godparents must have received all the Sacraments of Initiation: Baptism, First Communion, and Confirmation.

Baptism Prep Class:
Parents and godparents are expected to attend a Baptismal Preparation class prior to the Baptism. Classes are held the last Sunday of each month from 1-3 PM (with a few exceptions) in rooms 44-46. Baptism Prep dates and register for the class here
